ADEQUATE STAFFING

INCIDENT RESPONSE PROTOCOL

Purpose

This protocol outlines the required steps employees must follow when ANY incident, injury, hazard, or near‑miss occurs at a client worksite. Immediate reporting and proper documentation protect the employee, the client, and Adequate Staffing.

A. Immediate Response

  • Ensure scene safety — Stop work immediately and move away from danger

  • Provide first aid — Only if trained and safe to do so

  • Call 911 if needed — For serious injuries or emergencies

  • Notify client supervisor — Report incident immediately

B. Internal Notifications

  • Notify Adequate Staffing management — Call or text immediately

  • Notify insurance carrier — If required by incident type

  • Notify Workers’ Compensation carrier — For any employee injury

C. Documentation Requirements

  • Complete incident report — Must be submitted same day

  • Take photos of the scene — Only if safe and permitted

  • Collect witness statements — Names, contact info, and statements

  • Record time and location — Exact details required for insurance

D. Follow‑Up Actions

  • Submit WC claim — If employee injury occurred

  • Schedule follow‑up medical — As required

  • Review safety procedures — Identify root cause

  • Implement corrective actions — Prevent future incidents

E. Restrictions After Incident

  • Employee may not return to work until:

    • Cleared by medical provider (if injured)

    • Cleared by Adequate Staffing management

    • Client approves return (if required)

F. Zero‑Tolerance Violations

Immediate removal from assignment may occur if incident involves:

  • Horseplay

  • Working under the influence

  • Ignoring safety rules

  • Unauthorized equipment use

  • Violence or threats
